RHC COST REPORTING FOR 12/31/2023
February 4, 2024: It is that time of year again. This cost report season looks to be the most demanding yet. Due to significant modifications within the Rural Health Clinic (RHC) program introduced by the Consolidated Appropriations Act, 2021 (CAA), there has been a notable increase in the National Statutory Upper Payment Limits. These adjustments have escalated the per visit reimbursement rate to $139 in 2024, with an increase to $190 by 2028. Furthermore, the program has expanded to include various services that do not fall under the All-Inclusive Rate, necessitating their exclusion from the cost-per-visit calculation. These changes underscore the heightened influence of cost reports on financial settlements, both positive and negative, as well as the increased complexity and time required for their preparation. To ensure the timely and accurate completion of your cost report, we strongly advise that all requisite information be uploaded to your designated portal by May 1, 2024. Failure to meet this deadline may compromise our ability to guarantee the timely filing of your report.
To help you accumulate the information needed to file an accurate and timely cost report we have provided the following resources and will have a webinar on February 6, 2024 to go over the requirements. Provided below is a 3-page Cost Report checklist, a 24-page checklist and supporting workpapers that should be completed for us to have all the information needed to file the cost report, or if you prefer an Excel file to complete the workpapers is included as well.

Cost Reporting for Rural Health Clinics - What is needed to file an accurate and timely cost report

Note on RVU Factor (Discussed during Q&A Section of the Webinar): For every visit by a physician, the average work RVU is 1.67 and for APPs, for every visit the average work RVU is 1.38. Multiply one visit by the RVU factor (by provider type) to get the RVU proxy/equivalent. For example: if “Doctor A” had 1,000 visits it would translate into 1,670 RVUs for her; and if “Nurse Practitioner B” who had 1,000 visits that would translate into 1,380 RVUs for him. There is a difference between doctors and APPs because generally speaking, the former provides care to patients with more complex conditions.

Joe Lampard is Vice President and partner at HPSA Acumen. With over 15 years’ experience in analyzing health professional shortage areas, Joe has extensive knowledge about Health Professional Shortage Area (HPSA) designations, Medically Underserved Area/Population (MUA/P) designations, and Rural Health Clinics.
In September of 2021 HRSA completed an ‘Update 2.0’ review of all primary, mental, and dental HPSA’s throughout the U.S. Over 40% of the designations went into a ‘Proposed For Withdrawal’ status. This effected Federal loan forgiveness, RHC relocation and setup, as well as Medicare HPSA bonuses in 2022. Introduction to the Rural Health Clinic Program
In this webinar, Mark R. Lynn, CPA (Inactive), CRHCP, CCRS will present information to providers interested in becoming a rural health clinic (RHC) or RHCs new to the RHC program. It will provide basic information on conditions of participation, reimbursement, billing, and cost reporting. We will discuss advantages and disadvantages of the program and identify resources to help you become RHC compliant and obtain RHC reimbursement from Medicare and Medicaid. This 45-minute webinar is designed for people new to the RHC program or have become interested in converting their medical clinic into a rural health clinic under Public Law 95-210. We will discuss the nine conditions of participation, some basics of Medicare and Medicaid reimbursement, provide a timeline for certification, and provide some basic benchmarking information from provider based and independent RHCs.

The CARES Act Provider Relief Fund Payment Attestation Portal is now open. Providers who have been allocated a payment from the initial $30 billion general distribution must sign an attestation confirming receipt of the funds and agree to the terms and conditions within 30 days of payment.

On Saturday we recorded our Telehelath webinar on Medicare Part B Fee for Service. It is rather long and includes information on Modifier 95, and new place of service guidance, the use of the CS modifier (not just for Telehealth), and new guidance for enrollment for providers working from home (spoiler, you do not have to call and add their home address to their Medicare Part B enrollment). We also have a lot of links to cheat sheets, tool-kits, and other information you need to start a Telehealth program. While this webinar goes over Medicare Part B fee for service information and is not directed at RHCs it will be helpful to RHCs that have non-RHC time or hospitals with clinics that do not have the RHC designation. We still do not have word on the new RHC billing guidance as a distant site, but as soon as we do we have a great line-up on standby to get the information to you as quickly as possible.
RHCs got a nice surprise on Friday. A check from the federal government. To help you understand the meaning of this CARES Act Relief Fund money we have produced a “pop-up: webinar that lasts 10 minutes and will explain the monies and how to account for them.
